ATTENTION INCOMING 6TH GRADERS!

Students MUST present an updated blue card with proof of the state mandated Tdap vaccination BEFORE you may pick up your class schedule and get a locker for the fall term.  Students who do not present an updated blue card WILL NOT be allowed to begin school on August 5.

Dear Parent or Guardian,

Welcome to East Limestone High School, home of the Indians! I am here to serve the students and faculty by preventing illness and promoting health in the school setting. If your student has a medical condition that I need to be aware of, please contact me.

Listed below are Limestone County School Policies regarding illness:  

Notification-Parents will be notified for severe head injuries, temperature of 100* or greater, and suspected communicable diseases. Parents are not routinely notified for minor injuries or visits to the nurse. You will be asked to pick up your student if he/she has a temperature of 100* or greater, is vomiting or has diarrhea.

****Students must be free from fever, vomiting, and diarrhea for 24 hours without the help of medication before returning to school****

Medication-If your child requires medication at school the physician must complete a Provider/Parent Authorization form. These forms must be signed by the parent as well as the physician in order for the nurse to administer medication. You can obtain the Provider/Parent Authorization form from the school nurse or from the Limestone County School Handbook. Parents must bring the medication to the school along with the complete parent/prescriber form. Students are not permitted to carry medication unless authorized by their physician. Paperwork must be completed each year regarding any medical needs.
